Combo Box

I have set up a form for data entry that has multiple fields with the combo
box option of GOOD-FAIR-POOR. When I enter good in one field, it changes to
good in all of the fields. I want each field to be independent of each
other. (e.g. Field1 =Good; Field2 = Fair; Field 3 =Poor.

Perhaps all three combo boxes are bound to the same field in the table?
