Combo Box Problems

I have created a combo-box that displayes 5 feild records.

"LastName" "FirstName" "MiddleName" "City" "Address"

I would like to pick from that list a Lastname and have that fill out a form.

If I have a lastname Jones and the city Riverside then
Go and select another Jones in Anaheim the combo-box will only allow Jones
in Anaheim to be selected and thus fill out the form. If I click on Jones in
Riverside, it will not select Riverside and fill the form, it only allows the
click on the Anaheim record.

I hope I'm explaining myself correctly.

I am guessing LastName is not a Unique Key. In order for you to what you
what you want the first column, which can be hidden needs to be a Unique
Key.
