Collating Survey Results

I have been given the job of collecting together the results of evaluation
forms for courses (ie, those questionnaire thingys they give you at the end
of a course which give you the opportunity to rate the course on a scale of,
say, 1-6).

I thought Access might be the best program to tackle this (if it's not,
please advise!) but I'm not sure how best to go about it.

I wondered if anybody knows of any websites/books/other resources that give
examples of this type of database so I can get some ideas? Please don't say
go and buy a good book and start learning Access from scratch. I've had that
advice before but I already know the basics of Access. What I need to see is
an example so I can amend or remodel it to make my own database.

If anyone has any hints or tips at all on this topic I'd be very grateful to
hear them.

Duane Hookum has generously offered a top-notch Access sample
application designed for just this purpose. Download the appropriate
version of AtYourSurvey from:

Note: Access2000, 2002 and 2003 can all use the 2000 version.
