If you want some improved interface convenience for working with the MSFT
HHW, and less rtfm'ing, the good solution is in payware:

Very inexpensive($50) for that category: help applications can often run
hundreds to even thousands of dollars. I almost recommend FAR just as a
start point. Use it as an eval. It is uncrippled, and it is nagless for
your first 60 days.

Btw, even if wanting to stay pure freeware, still worthwhile to visit their
site and read some of their helpful docs about making CHM.

There is a strategy I haven't looked into. As I've not tried Abee CHM Maker.
It would go like this:

[1] First run Abee CHM Maker to make a CHM. If you find it easier/faster
than using the HHW.

[2] Then run HHW. Do a decompile on the CHM. Then choose new project, and
point it at your folder of extracted files. Then add the other features.

You'll need to consult the manual. For instance, after you tell HHW you
want an Index, you might find you're just staring at each other blankly.
That operation is esoterically hidden in a little icon of a key symbol,
called add keyword. From that dialog, you can click on files, and get
things inserted.

spent more time with HHW. Instead of cheating, with $FAR....]
