checkboxes

Hello:

I know that the wingdings font symbols can be used in Word to insert check
box symbols.

But, we have box symbols in a survey form that we created and we want to
have the user literally click the mouse inside the boxes to check them off to
complete the survey.

What can we do in Word in order to allow for these checks (checks for the
check boxes that we have created)?

There are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Take a look at the Checkbox template available at
http://www.addbalance.com/word/download.htm#CheckboxAddIn for an exploration
