Check box opens text field

Hi,
I am trying to setup a Word file that is locked, but has a check mark box
that can be selected. Once the check box is selected, a field will open up
where someone can type in their name, for example. I have tried searching but
can't find any assistance in how to do it. Please help.

What you are talking about is what Word calls an "online form." Check this
