check box not showing in outlook

I created a template in word 2003 that has a check box form field. I check
it and can see it in print preview, but when I receive the email the check
box is empty. Any ideas would be great. THanks

THe check box appears in word, but not in when I receive the email in
Outlook. I thought this is where I should post the question. Thanks
