Changes are not accepted when I save and close my file

I am working on a document created by someone else. This document contains a
table of contents and an index. Once I've made my changes, I saved the file
and closed it. When I opened it again, the changes are made are not there and
it's back to the original document. It's as if I never made changes at all.

What is causing this?
 
Did you receive the document as an email attachment? And did you
open the attachment directly from the email, make your changes,
and save? Then your changes are not saved to the email
attachment, but to a copy of the attachment that may or may not
be saved (depending on which email program you use) in a
temporary folder when you close the modified document.

Instead of opening the email attachment directly from the email,
you should right-click on the attachment, save the file to a
folder of your choice on your hard drive, and then
open/modify/save the file on your hard drive.
