Can't print return address on envelopes

I am using Word 2000. I was able to print my return address on the top left corner of envelopes, but now it won't print. I believe it is set up properly. Help please!

Under Tools > Envelopes and Labels, is the Omit box checked?

Thanks for your reply, but, no I haven't checked the 'omit' box and the correct details are in Tools/ Options/User information.
