Can't create new internet connection using username/password

My internet stopped working recently and now I find
myself needing to set up my broadband connection using a
user name and password. but when I go to make a new
connection, the options for connect using dial-up and
connect using broadband that requires user name and
password are diabled so I can't select them, I can only
choose broadband that is aways connected and that won't
work. If this has been posted about before, could
somebody please point me in that direction. Any help
would be appreciated.

The usual fix is to re-apply SP1.
