Can't add COM Add-in, but no error message

Strange problem here... Seems like it might be a permissions
issues. I can't quite nail it yet, though. Still trying to
find out what's causing this, but I wanted to check if anyone
else has seen this before.

I have a COM Add-in written by someone else. The COM Add-in
is installed on an XP machine by the local Administrator
account. Then a regular user logs in and runs Outlook 2002.
The user attempts to add the COM Add-in:
- in the COM Add-ins dialog box user clicks "Add..."
- navigates to the file and selects it
- click OK to go back to COM Add-ins dialog
- the COM Add-in entry DOES NOT show up in the window
under "Add-Ins available:"

No error message. Has anyone seen this before? Any ideas on
where to look?

Does the Add-in create an error log in the application folder or in the
Windows Event viewer? Does the Add-in Setup program create a setup log? You
might be able to determine why the Add-in does not load from these logs.
