Can you have a field on a form that can take images and tables?

I am creating a form that contains a table. I have the usual text entry
fields but on my form, I want a large area where the user filling in the form
can paste and image, add a table or write some free text. Is this possible
and if so how do I do it?

I want to still be able to lock the rest of the form so that the formatting
and tables cannot be changed.

Any suggestions?

You can divide the document into sections and then leave one section
unprotected where users can add text freely (from Word 2003 Help):

On the Tools menu, click Protect Document.

In the Protect Document task pane, under Editing restrictions, select the
Allow only this type of editing in the document check box, and then click
Filling in forms in the list of editing restrictions.

To protect only parts of a form from accidental changes, click Select
sections, and then clear the check boxes for the sections you don't want to
protect.

Note To protect only parts of a form from accidental changes, those parts
must be in separate sections. (On the Insert menu, click Break to create
section breaks.)

Click Yes, Start Enforcing Protection.

To assign a password to the form so that others don't accidentally change
the form's display content, type a password in the Enter new password
(optional) box, and then confirm the password. Users who don't know the
password can still enter information in the form fields.

Thank you very much - starting a new section worked a treat.

Is it possible to use fields in a header?

No, text form fields cannot be used in the header. But there are other
options, depending on what you are trying to do. For example, if you
want to repeat data from a particular form field in the header, you can
format the field using a unique paragraph or character style and then
insert a STYLEREF field in the header.

On the other hand, if you want a field that appears to be in the header,
put it in a frame and position the frame on top of the header.
