Can someone help me create a template?

2 parties

1 party is the complaining party, the other party is the defending party

I need to track, the complaining party company, complaint, defending party company, and resolution, plus any notes, date, show name, action taken, follow-up(flag?), payment complaining party, payment defending party.

I'd like to see any past issues, in the db, show up on the screen as well. If possible.
this is to see any repeat offenses.

Also, the parties could switch places, one time they could be defending, another they could be complaining. Maybe a flag of some sort?
