Can I time/date stamp a read-only section of a Word document?

Hi:
I would like to have a word document that I add to periodically. I would
like each addition to have a time/date stamp and for that section be
converted to read-only at that time.

The reason for this is that I would like to use a Word document as a patient
chart for writing psychotherapy progress notes, since this can be sync'ed
with my Palm. However, current federal regulations insist that each entry
into the chart should NOT be able to be modified at a later date.

I hope that I have explained my problem sufficiently.

Realistically, there is no way to do this in Word.
You can make it so that it is difficult to change, so that you would have to
deliberately defeat the protection to change, but not so that it could not
be changed. Suggestion: Save your file as a series of pdf files for
historical backup or freezing. Also not impossible to change, but impossible
to change from within Word.
