Can I create a new document with a hyperlink using template?

I have a document with a form in it that I want users to be able to fill out.
But, I do not want users to change the form in the document, and I don't
want them to print the whole document just to get the form. My thought was
to provide a hyperlink that would create just the form for them on their PC
so that they could enter the required information, save and print it from
their PC. I can make the hyperlink point to the Form Template, but it opens
the Template instead of opening a new word document using the template.
