Can document be protected?

I am trying to create a questionnaire to be emailed to several clients. I
have saved my creation as both a document and a template. Is there any way
to "protect" the document so that clients can answer the questions but not
save or change the questions?

Ultimately any document that you allow someone to see can be edited. Forms
protection is more concerned with accidental changes rather than protecting
your work from plagiarism.
