Does anybody know how I can add a little calendar icon next to a date field
in a form? Basically, I want to be able to click on the little icon, which
then brings up a calendar, I then choose a date and that is filled in the
field. Like the ones you find on travel agency websites

My personal favourite is what Stephen Lebans has at
http://www.lebans.com/monthcalendar.htm It may look complicated at first
blush, but it works extremely well (like everything Stephen's got at his
site...)
