Cache full?

There is a Microsoft Knowledgebase article
(http://support.microsoft.com/default.aspx?scid=kb;EN-US;260650) that states
that when trying to save images from the web, UNTITLED.BMP is the only
option offered.

The resolution they offer (and apparently works) is to delete Internet cache
because "This issue may occur if your Temporary Internet pages cache is
full."

How can Internet cache become full? IE is supposed to remove the oldest
files to make room for the newest files. Now, if there is a file corruption,
this makes sense.

Can someone explain what Microsoft means when they say Internet Cache is
Full?

Is it that the maximum number of filenames allowed in the directory has been
met or exceeded that of the file system?

I think they mean that the replacement algorithm needs to be invoked
and evidently that gets in the way of normal processing.

FWIW I use CacheSentry which does its flushing
independently of the IE and at a smaller maximum.
Hence IE's replacement algorithm is never invoked
and I never see that problem.
