When installing my VB .NET program it is NOT listed under the START -
All Programs list of programs.

I have tried adding the "Program Files Folder" to the "File System on
Target Machine" but not sure what other options I should select OR if I
should add another "folder" to the "File System on Target Machine".

Advise on what options I must select during the Build process is much
appreciated.

Try adding the User's Start Menu or User's Programs Menu folder, depending
on which folder you want to add the icon to and then add a shortcut to the
application exe to that folder.
