When did the problem begin?

Could you have a defective data cable?

Is this drive SATA, IDE, SCSI?

Go to the hard disk drive manufacturer's web site,
download & run the diag utility. Your system will
probably need a floppy drive.

Put the drive in another system. Does the problem occur?

Install a controller card into a PCI slot. Does the
problem persist?

That should keep you busy for a day or so.
