blank form for message recipient

I created a custom form on the Edit Compose Page as well as the Edit Read
Page. However when testing the form and sending it the recipient receives
the form with only the From, To, Subject and one field in the form with text
in them. The one custom field with text is same as the subject. I set up
the field so that the subject line would populate the field. Why is the rest
of the form blank?

Exactly what is blank on your form?
