BCM: "Object could not be found"

I'm finally getting around to trying Outlook with Business Contact Manager
(2003) again and having some problems getting it set up. I thought the
problem could be resolved by uninstalling and reinstalling it (with the
Update and Service Packs 1, 2, and 3), but I'm having the same issues:

1) When I click on the Activities tab of the default form, I get an error
"No folders are defined for the Activities tab of contacts in this contact
folder. Right-click on the contact folder, and then click Properties. On the
Activities tab, set the folders that should be search for items associated
with this contact"

I follow these instructions, but no folder groups are shown. I attempt to
create a New folder group, but I'm not sure what I'm doing and every attempt
gives me the error "The operation failed. An object could not be found."

2) When I try to save a contact, I get the error "The item could not be
saved to this folder. The folder has been deleted or moved, or you do not
have permission. Do you want to save a copy of it in the default folder for
the item?"

If I answer "Yes," I then get an error "The operation failed. An object
could not be found."


Again, this is with a fresh installation of Business Contact Manager with
all the updates/service packs. I have created a brand-new Business Contact
Manager database and I am adding a contact to Business Contacts in Business
Contact Manager. (If I add a contact to Contacts [under My Contacts], I have
no issues.)

Any suggestions?

Inexplicably, BCM had been disabled. I re-enabled it by selecting Help >>
About Microsoft Office Outlook >> Disabled Items.
