Banner Sheet (Seperator Page) Windows 2000 Server

Due to HIPPA our company has decided to add banner sheets
that give the user name and time stamp to each print job a
user sends to the printer. How can you do this with a
Windows 2000 Server? Also if I send a job that is 6 pages
will it print a banner sheet for each page, as I would
only want it to print one banner page?
