Until a couple of days ago, whenever I'd send my wife an e-mail from
work, I'd start typing her name and Outlook 2003 would fill the rest
of her name in. Now her name isn't on the drop down list, although
it is on my Contacts.

I don't know what changed. Why isn't it autofilled anymore?
 
Because Outlook lost its connection to your autocompletion cache, which has
nothing to do with your Contacts. You'll probably have to create a new
profile to get it working again, although you'll be creating a new cache all
over again.

Other names auto-complete, including some that are only in the
Contacts.

You are mistaken. Autocompletion will only use names of previous recipients,
not Contacts.
If you haven't lost all your autocompletion data, then why do you need a new
profile? You'll start all over again for sure if you do that.

of the autocompletion cache to see whether or not the entry still exists.
Here's one: http://www.nk2.info/ .

If you elect to try a new profile, you may be able, with Outlook closed, to
rename the existing NK2 file to match the new profile name, reconnecting the
old cache with the new profile.

That shows a bunch of names and e-mail addresses, but not my wife's.
It has an option to add one, but that doesn't change the Outlook file.
(It can export to be imported into contacts, but her name and e-mail
are already in contacts, they just don't autocomplete!).

I thought Auto-complete worked automatically - if I write enough
e-mails it would be there. Hers auto-completed fine until this week,
but now I have to type in her whole e-mail or select her name from
contacts by hand.

I'd like to keep all of my other auto-completes.

All you should need is to send a message to an address once to have Outlook
retain it.

It may be time for a new mail profile. You can then rename the NK2 file to
match the name of the new profile and it should work again.
