AutoCorrect - Can;t add some words.. Why?

Why is it that when I entered some words in the Replace:
and With: box in the AutoCorrect dialog box, I can't add
them? E.g. the Add button is not enabled..
 
It could be that the strings have already been entered. What are you
entering in the two fields?
