attach a file in a Word form

Is it possible to create a space in a form in Word in which files can be
attached (so show up as an icon, not paste the whole document)?

try

from the menu bar

insert
object

create from file tab

make sure show as icon is ticked

then browse to the file

You might want to post the question in a word newsgroup for further help as
it's not strictly an XP question
