Append Query is not working

I created a Select Unmatched query using the query wizard which works
perfectly. I am now trying to append this unmatched records to the main table
(Company Profile) by converting the query to an append query.
When I converted the unmatched query to append query, it took the Souce CP
(Source Table) and the append to shows the fields in the destination table.
When I click the run button, an error showed "Duplicate output Destination
[Account Number]"

How come? Please help me.

Same field twice. Open query in design view, click on VIEW - SQL View,
highlight all, copy, and paste in a post so it can be examined to tell you
the exact error.
