Adding items to ShellNew?

How do I add an item to Explorer's "New" context menu? I've
tried adding ShellNew keys in various places in the registry with
no luck. Does this require some type of shell extension or
something?

Well, that has nothing to do with it. There happens to be an article
on that site explaining how to remove items from the "New" menu, but
not how to add them.
