Adding attachments to a form

I have created an input form that allows me to add new employee profile data
into a table. I would also like the ability to attach the excel request
template that is submitted to me to this form. That way I have the
supporting document when I go back and view the form at a later date. Is
this possible? If so, please point me in the right direction.
