Adding additional recipient to reply

I recently downloaded the Vacation Request form off of slipstick.com.
I've been struggling with this problem for a day now. Does anyone
know if there is way for an additional email address to be added to
the reply to message that the supervisor would return to an employee
that would signify whether their request was accepted or rejected?
For instance, the acceptance email would be replied to the original
sender as well as CC'ed to another person so that it could be added to
an online calendar application when the user hit the Approve button.

Sure, you should be able to modify the code to do that. It's pretty straightforward stuff, working from the Item_CustomAction event handler.
