Add table programmatically

I have a frontend and backend database, which I distribute to clients
( a dental management package www.VisualDentist.com). I need to
programmatically add a new table in the backend using a new frontend
which I will distribute. I believe you need to have admin rights to
add a new table in the backend, which implies a need to directly open
the backend.
Are you aware of a way I can add a table using the frontend?
