Add online survey

How can I add a survey to one of my pages in FP2000? The
kind that allows a user to select from a list of choices
and then provides the results once the've casted their
opinion/vote. Any help would be appreciated.

Do you want their answers stored somewhere, like in a database, or do you
just want to tell them how they did? Here's a JavaScript quiz that tells
them how they did, but does not store the results:
