Add bookmarks in header/footer

I see several KB articles about a hotfix that enables bookmarks in footers; I
see several articles in this newsgroup for going to bookmarks in footers, but
I need to know how to insert a bookmark in a header/footer; more specifically
into a table in a header or footer.
