Activating windows after reformatting

We reformatted the computer. The windows activation is
acting up. Its popping up saying we need to activate it
but its already activated. What do we do next?

Windows XP will have to be activated everytime after a repair, reinstall or
full install, there is no limit on the times it can be activated.
