- Card is defective
- Computer is overclocked
- Card is not completely into the slot
- Power supply not good enough

Second hand:
Its been overclocked at some point and either the GPU or RAM has fried.

New:
The heatsink hasn't been fitted to the GPU properly, its overheated and
has fried.

Either way sorry to tell you this but its broken and unusable.
