The current version of VSTO is 2005, not 2003. Please see the following Web

However, the next version of the Access Developer Extensions (with the
Access 2007 Runtime) is going to be free.

My understanding from the
literature is that these tools will give me a licence to distribute my
app.
Is this correct?
No. These tools (specifically the Access Developer Extensions) give one the
unlimited license to distribute the Access Runtime (a cut-down version of
the retail version of Access). One may distribute a custom developed
Microsoft Access database application (mdb, mde, mda, accdb, or accde)
_without_ the Access Runtime, and therefore, without the need for the
Runtime license. Hence, if your customers have the retail version of Access
2003 already installed, just distribute your database application (unless it
requires other licenses -- but that's a separate issue).
