Visual Studio debugger

I have an asp.net application written in C# and am having trouble with
the debugger hitting the breakpoints. When I set the breakpoint, I
get "?" tooltip and it says that the breakpoint will not currently be
hit. But occassionally, when I first open Visual Studio, it will hit
it. Very frustrating. I'm using Visual Studio 2003, Windows XP.
Any help will be greatly appreciated.

I am in debug configuration (I really wish it had been that simple).
Looking on the web, this problem seems fairly widespread, with a
variety of causes.
