Survey Questions

Hello,

I'm looking to create a simple basic survey with options of (M/C, T/F, Y/N).
I would like to have atleast five questions and would like to send the
survey via outlook 2003 to my co-workers. Then, I would like to see their
responces. Any idea? Thank you.

Do you want to build a survey in a Microsoft Access database? There's no
need to "send" it to them, if they all share the same LAN. The answers can
be stored into a backend database on the network.

The boolean Yes/No field should handle your binary choices listed.

What I'm trying to say is that I would like to create an htm form in access
and send it an attachment in outlook. Thanks
