outlook 2003 survey form

i want to create a survey form for my customer, who are using outlook 2003
and are on same exchange server. what i want to do is send a message with
five questions. the answers to these questions are selected from given
options(drop down or option buttons). after selecting the options when our
customer clicks on send button the values which were selected comes to my
inbox as a mail message.

how can i accomplish this.

What have you already tried? I simply would create a word document with
fields and dropdown lists and send it as e-mail.

i dont want to use attachments. i want the answers of the survey questions to
come as message. i have already created outlook form and i can see the
answers on my system. what i want to know is how can i send that form to the
user. do i save that form in organization forms folder of exchange server
but in that case how do i send the form link to the user.
