Submit button still not working....

I have tried everything; read through all the threads; called my website
hosting company; and the 'submit' button is doing nothing. I have installed -
uninstalled - and re-installed FP Extensions. So, what am I doing wrong or is
my web hosting company doing something wrong? The form is at
www.spiritofthewestmagazine.com/questionnaire.htm

You have a closing '</form>' tag at line 196. Remove it.

Thank you. Took it off. Now, the submit button clicks but I am getting an
error message.

The no northbots error means there's no runtime bots...in otherwords your
hosted site does NOT have FrontPage Server Extensions (FPSE) installed or if
installed they're not working. In your case the FPSE are NOT installed...so
the form cannot work.
