Senders name in forms

Hi, somehow last year I set up a form (in MS FrontPage) for a Malaysian
client and the senders name came up in the inbox plus the subject as
determined by myself. Following some price changes on the form, it now shows
the sender as anonymous but subject line is OK. I cannot remember what
combination of settings I used in form properties to get this same effect.
Now my Malaysian guy is going potty because I cannot get back to the old way
with senders email address showing in my clients inbox. Any help to get back
there would be appreciated. I have not got the skills to write a CGI script
form handler, so what to do now????

The setting is probably Form Properties, Options, Saved Fields, Username.
However, this only provides data if the Web server requires a login to access
the page.

Otherwise, you have to add a text box and ask the visitor to hand-type their
e-mail address.

This would be a good time to restore a backup of the original page, eh?
