Question using Survey Form from slipstick.com

I have downloaded the survey form posted at:

http://www.slipstick.com/dev/olforms/survey.htm

When I create a survey and send it off to a recipient (myself), I do receive
the email but can only see the form shell but there are no questions in it.
I am using Outlook 2003 on an Exchange Server but I am doing the testing by
publishing the form in my Personal Forms Library, creating a new survey from
the oft file then sending it to myself. When I open it, I can see the shell
only without the questions I created. I was wondering if anyone could tell
me where I am going wrong.

You must create the form from the published form, not an .oft file.
