Form Survy

I want to send my users a form with a survey. I can create the form, but how
do I send that out so that when the user opens the form they see my survey,
answer the questions, hit reply and it comes back to me with the answers?

Outlook version, including the build number from Help | About? Is Exchange your email server? If so, can you publish to the Organizational Forms library?

I do have access to the Organizational Forms Library.

I have downloaded the sample from Outlookcode.com (I see some posts from you
there as well). It works pretty good. I need to add a text field to my yes or
no answers and I am not much of a programmer.

Is there any easy way to add the text field?
Or, can I create a form (I can at least make it look like I want) and then
send it to my users to fill out and return without loads of programming?

Scott Bradley's sample from http://www.outlookcode.com/d/forms/survey.htm is pretty self-explanatory, I think. Once you publish it and then create a new item from the custom form, it should walk you through the process of setting up the survey questions.

If you want to create your own form, start at http://www.outlookcode.com/d/forms.htm. You'll want to make sure that the form for the Reply action on the (Actions) page in design mode is set to your published custom form.

Alternatively, create the form, preaddress it, but don't publish it. Instead, save it as an .oft file and send that file to your users.
