Access Pop Up Forms in Access

I am a basic user of Access and require a pop up form to appear (Diagnostics) from pressing a button from a main form (Contact). I have the tables linked OK and use ContactID on both tables as the link.

My problem is that the Pop Up Form (Diagnostics) is not carrying through the data from the Contact Form. Any ideas?
