pesky toolbar won't stay bye-bye

Using Office 2003, with Win XP Pro SP2. I have one particular Word fil
that behaves different from all my other Word files. Every time I ope
this one file, a small floating toolbar (attached) appears on th
document. This only button on the toolbar is Exit Design Mode. If
click on that button, the toolbar disappears until the next time I ope
the file. Then, it reappears.

I don't recall why this toolbar ever appeared in this file in the firs
place. Anyway, how do I make it stop appearing every time I open thi
file

Edit: NOTE: I tried to upload an attachment, both directly from my P
and from my photobucket.com account. It's not working. I'm usin
Firefox. Do I need to use IE for this to work

OK, thanks. The ActiveX controls must have come from something
copied into Word from a web page
