PDF Creation Software

I need to be able to create some PDF's from items that have in the past been
created the old fashioned way, pen & paper, and photos.

What I need:

1) Scan in the blank form to some sort of file to use for all future PDF's
created. I then mask off "x" boxes where an X and ONLY an X can be placed, mask
off text box areas, and mask off an area for the next item. If it will accept a
PDF that was created by a form program as its source even better, but will scan
a blank if needed.

2) IMPORT JPG and/or other digital camera format pix files into SPECIFIED areas
of the original form.

3) A way to automate the above import, ie: I insert some sort of tag and it puts
the photo in from the SD card reader.

4) Ability to put "x" into pre-defined boxes for categories. Ability to put free
form text into defined text boxes.

5) Encryption and security features such as: password to change, disable
printing, disable text/graphics copying, disable commenting, disable text
editing, disable page addition. Would be a plus, but not required.

6) FREEWARE or low cost is a must. Re-read, free or cheap a must!

7) Must run on standard windows PC's

8) No Adbobe SDK or Acrobat Creator too $$$$ and not freeware.

9) Must maintain readability in the standard Adobe Reader and on all platforms
including Linux via the various readers.

10) Scriptable/automation for doing this for say 100 reports a day which may
have 1 to max 3 pages for each report and assigning file names in a format I
provide would be a real plus.

Foxit has a number of applications. Their reader is freeware, the
others are paid apps.

I was installing CutePDF writer and then the next thing I knew it
suggested that it needed CutePDF Writer Companion so I put that on and
as i did it went off and got some other software (prolly the spyware it
hints at on the web site).

Anyway, before it was over I have a pile of software and ISTR than there
was some nag to go and get more. It all got confusing (for someone of
my limited understanding anyway) so I went and ripped it all out.

The quotes say, "Excellent SIMPLE and PROBLEM FREE," so maybe I am just
far too thick but it was not simple.

IIRC floppy office does pdf creation, but I dont know more details
