Outlook Survey that returns respondant's results to Access Databas

I'm trying to figure out a way to email a survey to a group of people, but I
want to collect the respondant's survey results to feed into an Access
database.

I've tried EasySurvey, but it doesn't work the way I need it to.

Does anyone know if this is possible? If so, how? Any suggestions? Also,
if anyone does have a suggestion, will your way support the survey to have a
drop-down box? HELP!!!!

Do all the recipients use Outlook? Are they all in your organization and, if so, do you use Microsoft Exchange as the mail server?
