old e-mail address which keeps appearing

I have an old e-mail address that I am unable to get rid of.

I have:
Deleted it from my contacts.
Deleted it from all my mailing lists.
Deleted my .NK2 file.
Deleted and recreated my offline folders.
Consistency checked and ISINTEGed my Exchange store.
Deleted and recreated my profile.

It appears on different computers and in OWA.

However it continues to appear when I reply to the person it used to
belong to (though they send from their new address)
It also appears when I type the person's name and let Outlook complete
the address.

Any more ideas?

In what information store did this address originally reside?
What version of Outlook are you using and was it an upgrade from an earlier
version?
