Your problem is not likely the Windows XP Firewall as it only blocks
incoming traffic and Outlook does not require any ports be opened to allow
connections in the way you are using it. If the exchange server Could not
Send through port 135, that would be an issue. However, as you can hit
send/receive and everything functions I think you need to check your Outlook
settings.

Actually there is an incompatibility with ICF and Outlook in this area. The
basic idea is that the Outlook client will dynamically request a port on
which it will get the messages about new mail. This port will likely be
different every time you launch the program so you can't really open a hole
in the firewall for it.
We are addressing this in XPSP2 by letting you open holes for applications
rather than just static ports, however that doesn't really help you at the
moment.

Newer versions of Outlook have a registry setting that you can use to help
mitigate the issue (though I can't remember how) but again that also won't
do you much good.

I did a quick search on microsoft.com but didn't quickly see any kbarticles
that would help.

I suspect your easiest (only?) solution is to try and set your outlook
client to check for new mail on a short interval. (If I was at the office I
could walk you through this, but I'm at home using outlook express and I'm
not sure if the UI is identical).
