htm/html/ht*-filetypes have no icon when associated with IE!

A couple of times lately my html-filetypes have lost their icon. usually I
can just rebuild the iconcache and it will fix it, but this last time it
doesn't get fixed.

No matter what I do the icon for htm/html/any ht*-file associated with IE is
shown as the default "no-icon" icon (a folder with some icons). I have tried
setting other browsers as default browser in the hopes that IE would take
the association back, but nope - the check is set on "Internet Explorer
should check to see whether it is the default browser", but IE doesn't
realise Firefox or Mozilla have taken the role as default browser.

Mozilla and Firefox associates with the correct files fine and sets their
icons, but setting IE as default returns the icons to the wrong type.

Funny thing is that in Explorers' "Folder Options-File Types" list the icons
are wrong in the initial list, but correct in the "Advanced"-dialog.

What is going on? How can this be fixed?
